I, the parent/legal guardian, give permission for my child to participate in Royal Kings Baseball. I acknowledge the risks inherent in athletic activity, and I release and hold harmless Royal Kings Baseball, its coaches, staff, and volunteers, Silver Lakes Youth Baseball, and the City of Miramar from liability for ordinary negligence arising out of participation in practices, games, tryouts, and the use of the fields and facilities. I authorize emergency medical treatment for my child if needed.
